Form E-4 is known as the Social Security System Member Data Change. This form must be completed by the members of the social security system of the Republic of the Philippines.

Is Form E-4 Accompanied by Other Documents?

Yes, there is a set of documents that must be attached to the form. The attachments depend on the information you have changed. If you change the type of membership and you are self-employed, no documents are required. If it refers to non-working spouse, attach the copy of the Marriage Contract or Marriage Certificate. If you change the name or the date of birth, prepare such documents as birth certificate or passport, driver's license, health or medical card, firearm license card, voter's identification card, home development mutual fund (Pag-IBIG) transaction card, senior citizen card, TIN card, postal ID card, government service insurance system (GSIS) ID card and others. Moreover, you may be required to provide your alien certificate of registration, police clearance, life insurance policy, transcript of records, etc. Check the whole list of possible attachments in the instructions to Form E-4.

The form consists of 2 parts. Part 1 must be completed only by the members. You must provide the following information: social security number, common reference number, date of birth, tax identification number, full name, address. You must not fill in Part 2. It is aimed at the social security system.
